Aim
To provide an overview on medicines optimisation issues for adults at the end of life.

Intended learning outcomes
Having completed this workshop you will be able to:

• recognise the signs and symptoms that suggest a person is entering the last weeks of life
• understand the specific end of life needs of patients
• effectively manage the symptoms experienced by these patients
• support the use of subcutaneous medicines via a syringe driver
• convert opioid doses
• provide support and reassurance to family and carers, signposting to other services accordingly
• work collaboratively with other healthcare professionals to optimise end of life care for patients.

Please note that the ACP workshops are half day sessions and will run in the morning (10am – 1pm) and in the afternoon (2pm-5pm) at each venue. In total, there are ten ACP: End of life care workshops on offer. Please take care to ensure that you apply for the session that you wish to attend.
